SESSION RECORDING REQUEST FORM
Fill out this form and turn into recording services mailbox, in the main office. This form should be turned in at least 2 weeks before session date. We will, however, try to accommodate all requests. We will contact you by email or phone to confirm availability of recording services. There will be a $40 late fee for re-scheduling, relocating or canceling your recital with less than 24 hours notice.
RECORING SERVICES WILL NOT BOOK THE HALL OR ROOM SPACE FOR YOU. YOU MUST CONTACT THE MAIN OFFICE FOR SCHEDULING. DO NOT FILL OUT THIS FORM UNLESS YOU HAVE ALREADY RESERVED THE SPACE.
Recording will be billed at $25/hour. CD, cassette and DAT copies are available. Editing, processing and assembly will be charged at $25/hour.
