Recital Recording Request Form
Fill out this form and turn into recording services mailbox, in the main office. This form MUST be turned in at least 2 weeks before your recital date (you will be subject to a $20 late fee if request is late). We will contact you by email or phone to confirm availability of recording services. There will be a $40 late fee for re-scheduling, relocating or canceling your recital with less than 24 hours notice.
Cash or checks are accepted. Please make checks or money orders payable to "Michigan State University." Email recserv@msu.edu or call (517) 355-7674 with any questions or special needs.
